GENERAL RESPONSIBILITIES:
The Health Home Care Coordinator is responsible for the provision of all aspects of Health Home Care Coordination services for children. The position requires meeting the requirements of the lead Health Home, DOH and WYH to address the behavioral, medical and social risk factors necessary to meet optimal health goals while avoiding hospitalizations.
SPECIFIC RESPONSIBILITIES:
· Completes required reports for the Lead agency, NYSDOH, WYH and, as assigned, by Supervisor.
· Assists children and their families to achieve outcomes as required by the Health Home and NYSDOH.
· Maintains assigned caseload of patients in all phases of Health Home care coordination and maintains a productivity rate as established by Supervisor.
· Provides outreach and active engagement activities for all patients in assigned caseload.
· Completes CANS assessments for program participants as directed by Supervisor
· Facilitates entry to the Health Home for children and their families identified by WYH providers, other community based providers and the lead health home.
· Participates in educating and assisting care managers as appropriate in performing their job responsibilities and works as a strong team player.
· Engages and mobilizes family strengths and appropriate community resources to assist in goal attainment.
· Conducts client-specific advocacy related to supportive services.
· Participates as part of a multidisciplinary team in collaboration with Primary Care-Pediatrics and Behavioral Health providers.
· Utilizes children’s/SED care coordination activities and action board
· Completes all required documentation including progress notes, assessments and Care Plans in accordance with Children’s Lead agency, NYSDOH and WYH.
· Effectively documents patient information in the electronic case records and billing system set forth by WYH and the Children’s Lead agency.
· Attends meetings and trainings as assigned by Supervisor.
· Participates in Performance Improvement/Continuous Quality Improvement activities, as assigned.
· Demonstrates excellence in both internal and external customer service.
· Understands and is able to effectively communicate HIPAA compliance, corporate compliance and client confidentiality.
· Ensures and/or remains in compliance with local, state, and federal regulation, i.e. DHHS HRSA and NYSDOH, and all accreditation standards (e.g. Joint Commission and NCQA-PCMH).
· Adheres to the National Patient Safety Goals as defined by the Joint Commission and Whitney M. Young Jr. Health Center.
· Completes other duties as assigned.
Requirements
MINIMUM QUALIFICATIONS:
Associate’s degree in human services or related field with a minimum of two (2) year’s work experience in case management or case work with one of the following populations: persons with HIV/AIDS, women, children & families, persons with MR/DD, or substance users; Strong written and verbal communication skills; Provides excellent customer service and demonstrates a high level of cultural competency. Ability to adhere to strict confidentiality guidelines. Have a working knowledge of computers, particularly electronic health records and Microsoft Office program. Employees must have a clean license and valid driver’s license which will be verified annually. Proof of adequate auto insurance is required in compliance with NYS mandatory limits and coverage.
PREFERRED QUALIFICATIONS:
Bachelor’s degree in Psychology, Sociology, Social Work, or Nursing with at least one 1(one) year of related Health Home work experience in a medical setting. Bilingual (English/Spanish).

Salary range: $21.00 - $24.00 hourly
