Health Advocates Network Physical Therapist Long Term Care in Albert Lea, Minnesota

Health Advocates Network is currently seeking a Physical Therapist – Long Term Care to work at a facility in Albert Lea, MN . These are registry positions with our company.

Shift: 8:00am – 4:30pm

Rate: $66.00-$70.28/hr.

Description: Candidate must live 80 miles away from facility to qualify for highest rate. It is a 13-week contract but there's an option for longer term – 1 yr. 50% Albert Lea SNF & 50% Preston Home Health; Covering/helping at Austin SNF as needed. All skilled / rehab – post acute or long-term resident and outpatients. Flexibility in schedule for time off requested or to shift worked days: Normal is M-F could do Su-Th to have F off or 4 10s M-Th for F off etc. Productivity expectations 80-85% (includes evaluations and group/concurrent time in full). Therapy Supervisor on-site/ Lead Therapy Assistant on-site. Must have state PT license and active AHA BLS certification.

Job Responsibilities

Plans, organizes and delivers physical therapy programs that help clients/patients recover and improve their mobility, relieve pain, increase strength and prevent deformities. Conducts examinations, evaluations, and interventions patients who are affected by injury or disease.

Utilizes tools and techniques to administer physical therapy interventions following safeguards. Documents client's/patient's information for evaluation; establishes therapeutic plans and modifies them if necessary. Educates clients/patients and their family members about treatment plans/schedules and self-exercises to be continued at home. Knowledge of and utilizes appropriate age-related treatments and evaluations relating to the needs of clients/patients. Demonstrate and incorporate knowledge of current research into daily treatment of clients/patients. May treat individuals of all ages with varying diagnosis and disorders. The majority of work will focus on those receiving intervention in the post-acute, long term care and home health care environments. Will demonstrate documentation in accordance with departmental guidelines and meet all regulatory requirements. Demonstrates level of treatment associated with their specialty provided to their clients/patients. Follows and adheres to accepted clinical practice guidelines of professional organization. Exhibits flexibility with acceptance of variable work schedules/assignments. Participates in activities to promote the department and facility. Will be interacting with individuals outside of the department including but not limited to providers, case managers, third party payers, public organizations, companies/contractual partners, etc. May be asked to supervise assistants, aides, and students as they provide patient care, treatment, or other services. Will be expected to provide ongoing education and training to peers in the area(s) of expertise as determined by the department leadership. Performs other related duties as requested by manager. Establishes and maintains productive working relationships within and outside of own area. Identifies common patient safety risks within the organization and how they can be avoided. Analyzes and records the therapeutic progress of clients/patients; recommends necessary adjustments. Applies physical therapy tools and technologies according to a client/patient's individual case. Conducts the activities required in each step of the physical therapy process. Demonstrates knowledge of development and delivery of patient education for diverse audiences. Directs the implementation of the physical therapy process in a variety of service settings. Evaluates and modifies a physical therapy plan according to a client/patient's health status.Provides insights into the application of various physical therapy treatments in different cases. Safeguards clients/patients in the treatment process (e.g. during movement into/onto equipment). Identifies the importance of monitoring patient responses and reactions during therapy. Adheres to legal regulations and organizational policies when discussing patient health information. Seeks opportunities to serve as a mentor. Knowledge of the value of self-motivation and initiative; ability and willingness to seek out work and the drive to accomplish goals. Recognizes ethical requirements within own specialized areas of responsibility. Follows guidelines, regulations, standards and safety procedures in the therapy process.

Qualifications

Appropriate education level required in accordance with state licensure. Must be licensed in the state(s) of practice as a Physical Therapist. When applicable, may require valid driver's license. Obtains and subsequently maintains required department specific competencies and certifications.
