St. Mary's Healthcare RN Risk & Safety Specialist in Amsterdam, New York

Hours Per Week: 40 hour week

Hiring range: $65,769.60-$85,508.80

The actual compensation for this position will be determined based on experience and other factors permitted by law.

Responsibilities:

  • Conduct investigations through clinician interview, medical record assessment and environment of care analysis for assigned patient care events categorized as near miss, adverse or sentinel events.

  • Facilitates root cause analysis(RCA) meeting and develop patient safety improvement plans to address safety issues identified during RCA meetings.

  • Review incident reports daily using Incident Management system and work with departmental leaders for follow up and closure of potential safety concerns. Identify cases requiring peer review and assign to provider. Communicate access needs to incident reporting system to patient safety organization and optimize changes to risk module, as needed.

  • Work with clinicians and administrators within the organization to implement clinical system and process improvements that will improve patient safety, enhance clinical performance, reduce clinical errors and support a culture of safety.

  • Conduct educational sessions for providers and staff on risk and safety related topics.

  • Track and trends data and prepares risk/ patient safety reports and metrics for presentation at monthly Safety, Board Quality and Departmental meetings.

  • Actively participate in various committees representing departmental issues/ risk and safety concerns.

  • Round in patient care areas with accreditation specialist to promote event reporting and to facilitate communication of “lessons learned” from adverse patient care event investigations and analysis.

  • Facilitate mandatory reporting to NY State for sentinel events.

  • Collaborate with CDI to review all hospital acquired complications to determine whether event qualifies as a patient safety indicator.

  • Facilitate daily safety huddle utilizing daily readiness board. Complete required data entry of safety metrics to board daily.

  • Facilitate the submission for the Leapfrog survey. Analyze, benchmark, and prioritize Leapfrog survey data to identify opportunities and initiatives to promote increasing positive responses annually.

    Complexity of Work:

  • Within scope of job, requires critical thinking skills, decisive judgment, and the ability to work with minimal supervision. Must be able to work in a stressful environment.

    Reports to: Executive Director, Quality & Performance Excellence, Chief Nursing Executive

    Minimum Requirements:

  • Licensure RN professional license required

  • Bachelor’s degree or higher in Nursing preferred

  • Minimum of 2 years inpatient hospital experience

  • Experience with patient safety, risk management and/or quality improvement initiatives

  • Experience in process improvement methodologies, e.g., FMEA,SWOT Analysis or Lean methodology preferred

  • Working knowledge of CMS and Joint Commission standards.

  • Excellent oral, and interpersonal written communication skills

  • Strong organizing and data management skills, including word processing, spreadsheets, database and graphics

  • Experience working in electronic health records with Meditech Expanse preferred
