Description Lutheran Social Services of WI and Upper MI is currently seeking a Forensic Care Coordinator to join our Conditional Release Program serving Barron, Burnette, Chippewa, Dunn, Polk, St. Croix, and Washburn Counties! Conditional Release is a program that works with individuals with mental illness who have committed a crime but were found not guilty by reason of mental disease or defect. The program itself is there to offer community supervision and support to participants in the program. In this role we provide case management for individuals that have a diagnostic and statistical manual (DSM) mental health diagnosis. We meet clients where they are at in their recovery and within the community to help them access community resources that support their mental health and recovery. This role will be required to perform assessments, investigations, and screenings to determine entry and placement in the Conditional Release Program. This is a full-time, benefits eligible opportunity. Under direction, performs work involving the assessment, service planning, and implementation of the service plan for assigned individuals.
