Department Details

We are seeking a highly organized and proactive Patient Navigator to join our team. This individual will play a vital role in coordinating the services and activities within our cancer center, with a strong focus on community engagement, survivorship care, and program management. This is a hands-on, on-site role where you’ll be expected to support patients, staff, and programs while fostering connections within the community.

Job Summary

Coordinate, negotiate, educate, procure services for, and manage the care of, complex patients to facilitate achievement of quality and cost-effective patient outcomes. Knowledge of case management principles, professional practice standards, health and public policy, consumer health care issues and current trends in health care desired. Knowledge related to reimbursement, finance, allocation of resources, quality improvement, quality management and utilization management are desirable.

Qualifications

Bachelor’s degree required; nursing, social work, or counseling. Master's degree preferred.

Three years of experience and three years of management/leadership experience desired. Knowledge of case management principles, professional practice standards, health and public policy, consumer health care issues and current trends in health care desired. Knowledge related to reimbursement, finance, allocation of resources, quality improvement, quality management and utilization management are desirable.

Currently holds an unencumbered RN license with the State Board of Nursing where the practice of nursing is occurring and/or possess multistate licensure if in a Nurse Licensure Compact (NLC) state. Or currently holds a Social Worker license (SWL) with the State Board of Social Work Examiners where the practice of social work is occurring. Obtains, within 36 months, and subsequently maintains required department specific competencies and certifications.

A social work license must be in good standing in state(s) of practice:

In Iowa, must have one of the following:

Licensed Bachelor Social Worker (LBSW)

Licensed Master Social Worker (LMSW)

Licensed Independent Social Worker (LISW)

In Minnesota, must have one of the following:

Licensed Social Worker (LSW)

Licensed Graduated Social Worker (LGSW)

Licensed Independent Social Worker (LISW)

Licensed Independent Clinical Social Worker (LICSW)

In North Dakota, must have one of the following:

Licensed Baccalaureate Social Worker (LBSW)

Licensed Master Social Worker (LMSW)

Licensed Clinical Social Worker (LCSW)

In South Dakota, must have one of the following:

Social Work Associate (SWA)

Social Worker license (SW)

Certified Social Worker (CSW)

Certified Social Worker - Private Independent Practice (CSW-PIP)
