Description

Jet Health, Inc / First Choice Home Healthis seeking a dedicatedfull-time home health Clinical Schedulerto join our home health team. The primary responsibility of this role is to efficiently schedule patient/client services and coordinate with staff to ensure the delivery of high-quality care to our patients inBoise, ID,and surrounding areas.

This position could potentially handle a scheduling case load of 200+ patients.

Requirements

• Education: High School graduate.

• Experience: One year's experience as a home health scheduler highly preferred.

• Skills: Exceptional communication skills, experience with electronic medical records (HCHB preferred), ability to multitask in a fast-paced environment.

• Home Care Home Base experience highly preferred but not required

• Strong MS Office experience, Excel & Word required

• Willingness to help in other office functions as needed

• Team player with a can-do attitude

• Strong attention to detail

• Ability to follow direction and retain information

Essential Functions:

• Evaluation

• Schedules and appropriately documents patient/client schedules in a timely manner.

• Participates in coordinating care with management and patient/client interdisciplinary team.

• Provides effective communication to patients/clients, staff members, other health care professionals, and referral sources.

• Processes reports timely to ensure compliance.

• Responds to Agency and patient/client needs in a professional and creative manner.

• Promotes the agency philosophy and mission by presenting a positive image to patients, families, physicians, and community agencies.

• Demonstrates commitment, professional growth, and competency.

• Provides effective communication to patient/family, team members, and other health care professionals and maintains confidentiality.

• Performs other duties as assigned.
