Your Specific Responsibilities:

This is a paraprofessional position providing basic social services to individuals and/or families participating in the Healthy Families Program. The incumbent in this position will report directly to the Senior Human Services Program Manager of the Levy County Health Department.

Establish a trusting relationship with participating families by initiating and maintaining regular home visits and other contacts with the family after completing the HFF FSW Core Training.

Establish a Family Support Plan with goals, objectives and activities with the family and supervisor within 90 days of enrollment. This plan will be reviewed at each visit and updated at least every 5 months.

Maintain dependable, punctual work schedule, posting weekly schedule by 5pm Thursday with the supervisor.

Assist in strengthening parent/child interaction by providing in-home education, utilizing established program curriculum and by modeling appropriate behavior during each home visit.

Conduct all required assessments on time (ASQ, PSI, Safety), and maintain all required documentation and data submission to ensure program objectives are met.

Complete documentation of all home visits and contacts within 2 days and submit to the supervisor.

Meet with supervisor on a weekly basis for case management of all families and maintain communication with supervisor regarding schedule changes, concerns, or emergencies.

Attend regular staff meetings and case conferences as required. Participate in continuing education training, sharing knowledge with other team members and families.

Newly assigned families will be contacted within 3 days with continued creative outreach until family is engaged or closed (no longer than 90 days).

Demonstrate excellent knowledge of community resources, make referrals, provide follow-up with families to ensure needs are met, and submit required information for data entry. Maintain ongoing communication with community agencies and providers.

Performs other duties as required.

Internet; Microsoft Office applications; ability to work independently and multi-task; ability to effectively communicate

Qualifications:

Minimum – Willing to travel and have reliable transportation. Ability to communicate both verbally and in writing. Experience with children zero to five years old. Willing to work flexible hours including some weekends.

Preferred – Fluent in writing and speaking Spanish.
