Description

We are seeking a compassionate and dedicated Clinician for our SYNC (Serving Youth in Their Community) program. In this role, you will have the unique opportunity to provide services to youth and young adults, ages 0 – 21, in their home, school, and community environments. You will be impacting the lives of children and adolescents by supporting a transfer of learning across settings. The SYNC Clinician will complete assessments, develop treatment plans, and provide individual and/or family sessions as necessary. This position is part of a team of providers working directly with at-risk families.

Responsibilities:

• Assess and address the mental health needs of children and adolescents within their community settings, which include their home and school.

• Collaborate with families, community agencies, and other professionals to create supportive environments.

• Coordinate with schools, healthcare providers, and social services to advocate for the youth's needs.

• Develop and implement personalized treatment plans.

• Monitor progress, maintain thorough records, and adjust interventions as necessary.

Job Benefits:

Benefits for Full Time employees:

· 4 weeks paid vacation annually;

Requirements

You must have:

• Master’s Degree in Social Work, Mental Health Counseling, or a related field.

• Current licensure or eligibility for licensure in New York state is preferred but not required – LMSW (or Limited Permit), LCSW, LMFT, or LMHC (or Limited Permit).

• Experience in child and adolescent mental health services.

• Ability to work a flexible schedule.

• Valid driver’s license and reliable transportation.

You should have:

• Strong understanding of community-based interventions and resources.

• Excellent interpersonal, time-management, and organizational skills.

• Skilled in taking initiative and creative to foster engagement.

• Proficiency in clinical documentation.

• Commitment to ongoing professional development and learning.
