Essen Health Care Director for Health Home Quality Improvement in BRONX, New York

Job Summary: The Director for Health Home Quality Improvement is responsible for supervising the Health Home CMAs quality initiatives and outcomes under the Vice President for Health Home. This senior leadership role involves developing and executing strategies to improve operational efficiency and service quality for all Health Home CMAs. The incumbent is also in charge of leading the Health Home quality department to constantly assess CMAs compliance with health home policies and implementing proactive measures to maintain compliance.  Responsibilities include:1.    Administering and providing oversight of Health Homes quality department, including providing supervision to quality staff2.    Leading and overseeing quality improvement initiatives such as chart audits and case reviews to enhance patient care and operational efficiency3.    Ensuring compliance with all Health Home regulations and standards and implementing measures to address any areas of non-compliance4.    Collaborating with Health Home staff to develop and implement effective quality improvement strategies5.    Analyzing data to assess the effectiveness of programs, trainings, and interventions and reporting key findings to stakeholders6.    Managing complex quality projects from initiation to completion, ensuring alignment with quality improvement initiatives7.    Develop performance improvement plans aimed at increasing quality service delivery8.    Inform and facilitate training for staff on quality standards, processes and procedures9.    Establishing and maintaining strong relationships with external regulatory partners and bodies10. Performing other duties as assigned by the Vice President of Health Home  Education and Experience:MA, MS, MSW in Healthcare Administration, Public Health, Nursing, or a related field requiredPhD in a health-related field or DSW preferred5 years experience in roles that involve managing quality and compliance processes in a healthcare settingMinimum of 3 years leadership experience in a healthcare settingLicense preferred but not required (LMSW, LCSW, LMHC, CPHQ)Experience with quality improvement frameworks like Six Sigma, Lean, or other continuous improvement methodologiesStrong ability to analyze healthcare data and metrics to identify trends, gaps, and opportunities for improvementKnowledge of Health Home standards and practice preferredKnowledge of Health Management and Social Service programs preferred.Proficient with Microsoft 365 Office (PowerPoint, Excel, Word, etc.)Strong written and verbal communication skills Effective multi-tasking and time management skillsStrong organizational skills and the ability to balance multiple priorities in a fast-paced environmentProficiency in Spanish and other languages preferred
