ESSENTIAL JOB DUTIES Employee provides professional social work services to care management clients. Work involves evaluating the clients situation and ability to deal with it, developing a social history, psychosocial assessment, service plan and/or treatment plan and follow-up. Services may include basic counseling, support, and reinforcement in areas such as employment, vocational, medical, health, or behavioral issues. They may provide information to and confer with schools, natural parents, and foster parents on habilitation plans, behavior management, and other problem areas or needs. Refers clients and families to a variety of services to meeting financial, psychological, emergency care, emotional, or other needs. Assumes a case manager role to follow the client through agency and external services. Also provides Health and Behavioral Services (HBI) to pregnant women and may perform related duties as assigned.

KNOWLEDGE AND SKILL REQUIREMENTS Considerable knowledge of social work principles, techniques, and practices and their application to individual casework group work and community problems Knowledge of the psychosocial, socioeconomic, and behavioral problems and their treatment Knowledge of governmental and private organization and community resources Knowledge of laws, regulations, and policies which govern the program General knowledge of medical terminology, diseases processes, and their treatment is required in certain programs or settings Skill in establishing rapport with a client and applying techniques as assessing psychosocial, behavioral, and psychological aspects of clients problem Ability to establish and maintain effective working relationships with administrative superiors, members of caseload and their families, and with civic, medical, social, and religious organizations Ability to train or instruct lower-level social workers, students, or interns Ability to express ideas clearly and concisely and to plan to execute work

EDUCATION AND EXPERIENCE REQUIREMENTS Master's degree in a human services field from an appropriately accredited institution OR Bachelor's degree in social work from an appropriately accredited institution OR Bachelor's degree in a human services field from an appropriately accredited institution and one (1) year of directly related experience OR Bachelor's degree from an appropriately accredited institution and two (2) years of directly related experience OR An equivalent combination of education and experience *Bachelors Degree in Social Work preferred

SPECIAL REQUIREMENTS Must possess and maintain a valid North Carolina Drivers License

PHYSICAL REQUIREMENTS This is light work requiring the exertion of up to 20 pounds of force occasionally, up to 10 pounds of force frequently, and a negligible amount of force constantly to move objects; works requires stooping, reaching, standing, walking, lifting, fingering, grasping, and repetitive motions; vocal communication is required for expressing or exchanging ideas by means of the spoken word and conveying detailed or important instructions to other accurately, loudly, or quickly; hearing is required to perceive information at normal spoken word levels, and to receive information through oral communications and/or to make fine distinction in sound; visual acuity is required for preparing and analyzing written or computer data, visual inspection involving small defects and and/or small parts, operation of machines, operation of motor vehicles or equipment, determining the accuracy and thoroughness of work, and observing general surroundings and activities; the worker s subject to inside and outside environmental conditions and atmospheric conditions
