Private Duty LPN - PRN Days

Location:East Ridge, TN

Are you a dedicated LPN looking to make a real impact through one-on-one patient care? We have a high-acuity case for a vent and trach-dependent patient requiring exceptional skill and attention. If you’re passionate about bringing comfort and expertise into patients' homes, this role is for you!

Key Responsibilities:

• Provide direct nursing care to a high-acuity patient with vent and trach needs.

• Monitor and manage G-tube feedings.

• Perform routine assessments, administer medication, and respond to changes in condition.

• Collaborate with families to ensure patient comfort and health.

Requirements:

• Must have a minimum of 1 year of experience with vent and trach care.

Responsibilities

• Perform selected nursing procedures within the scope of LPN practice• Provide patient and family education according to plan as outlined by the RN• Monitor patient’s vital signs and make specific measurements and observations• Monitor and record intake and output, and report significant changes immediately• Observe, assess and record patient’s physical and mental condition and report any changes• Administer medications as instructed and ordered by the physician and instructed by the RN• Attend and participate in staff meetings and in-service education programs, as assigned

Qualifications

• Active LPN license in good standing• Previous home health, private duty, skilled nursing or hospital experience preferred• Ability to build rapport with patients and family members• Strong problem solving and critical thinking skills• Reliable transportation or valid driver’s license with current automobile insurance
