Job Title: Care Coordinator Location: Columbus, OH Job Type: Full-time Griswold Home Care of Franklin and Delaware Counties is a well-established and respected provider of non-medical home care services. We are currently seeking a Care Coordinator to join our team. The successful candidate will be responsible for coordinating and managing all aspects of home care services for our clients. Responsibilities * Evaluate clients' needs and match the right caregiver, both in skill level and personality * Manage the scheduling of a caseload * Focus on the quality of care and build warm client, family, and caregiver relationships * Provide counsel and loving support to clients, families, and caregivers to ensure an exceptional home care experience * Visit clients in their homes to ensure satisfaction * Earn additional compensation by participating in mandatory after hours “on call” rotation Requirements * Caring, empathetic, and compassionate individual * Strong verbal and written skills and a pleasant phone manner * Ability to build strong rapport with caregivers, clients and families * Creative problem solving skills * Organized and detail oriented; good documentation skills * Computer proficiency * Experience with WellSky software is a plus.
