    POSITION OVERVIEW

    Responsible for providing comprehensive counseling and support services to individuals struggling with substance use disorders.

    SCHEDULE AND PAY

    This is a full-time (40 hour) position. Clinic Hours are Monday – Thursday 8AM - 7PM, Friday 8AM – 5PM. Scheduling needs will be discussed during interview. Salary range: $18- $21 an hour, based on education and experience.

    DUTIES

  • Assists with providing patient care that addresses social determinants of health.

  • Assists with conducting assessments for social determinants of health (SDoH).

  • Provides direct services or connects patients to programs to address SDoH’s.

  • Educates patients on available community resources and how to access them.

  • Assists with providing case management to referred patients.

  • Conducts assessment of patients’ current social, financial, and emotional needs.

  • Assesses available resources and connects patients to programs.

  • Assists patients with formal and informal referrals

  • Assists with applications for medication, transportation, housing and other SDoHs

  • Assists patients with phone calls.

  • Conducts outreach and networking as necessary.

  • Coordinates referrals with community organizations.

  • Maintains and builds relationships with community resource agencies

  • Assists in maintaining and updating All Care Health Center’s Community Resource List.

  • Participates in outreach events as requested by supervisor.

  • Maintains documentation for all consultation notes, significant incident occurrences, phone conversations, etc.

  • Completes documentation within 24 hours

  • Maintains documentation consistent with applicable regulatory requirements.

  • Other duties as assigned.

    QUALIFICATIONS

  • Must possess a high school diploma or General Educational Development (GED) certificate.

  • A minimum of one year of experience providing case management to individuals, families, and groups preferred.

  • Must have working knowledge of county/state benefit programs, the health care delivery system, local health care providers and community resources.
