Description

Public Health Project Coordinator Data to Care

Under the supervision of the HIV Care Manager, this position is responsible for the implementation of the Data to Care model in the City of Detroit. The Public Health Project Coordinator - Data to Care develops all Data to Care policies and procedures. This individual leads community outreach and education initiatives, supervises the Senior Community Services Assistant – HIV/STI and Community Services Assistant , and conducts investigations/engagement of out-of-care individuals. The Public Health Project Coordinator - Data to Care works closely with the HIV Surveillance Team to track progress of clients and redirect programs, as needed. This individual also conduct s quality assurance activities, lead s quality improvement projects , and document s process evaluation and outcomes.

Examples of Duties

  • Work with the Michigan Department of Health and Human Services to manage the Not-In-Care list.

  • Design and maintain summary reports for staff use to meet state repor ting requirements, and support the development/evaluation of quality im provement projects. Participate in the preparation of grant reports and applications.

  • Coordinate data activities with partner agencies, including the Michigan Department of Health and Human Services epidemiologists and CAREWare staff.

  • Identify other entities that have relevant data and develop/ maintain relationships.

  • Present data and reports to stakeholders (e.g., planning council members, and Ryan White providers) through written, electronic , and verbal presentations.

  • Participate in Public Health Emergency Preparedness activities in real or simulated emergencies.

  • Other duties as assigned by supervisor.

Minimum Qualifications

  • Master’s Degree in Public Health or related field of study.

  • Three (3) years of experience in data management and statistical analysis.

  • Experience working with people living with HIV.

RETIREMENT BENEFITS

City Employees Retirement System

As a regular City employee you automatically become a member of the General Retirement System. This entitles you to a retirement allowance after:

• Completion of thirty (30) years of service;

  • At age sixty (60) if you have at least ten (10) years of service, or

  • At age sixty-five (65) with eight (8) years of service.
