The Amergis Division is currently seeking highly skilled LPN Nurses for the available position in El Paso, TX

Knowledge and Skills

· Proficient in case management processes and person-centered care practices.

· Excellent verbal and written communication skills.

· Strong analytical decision-making and judgment skills.

· Demonstrated ability to function as a clinical care team leader.

· Knowledgeable of all clinical resources available to patients, both inpatient and outpatient.

· Competent in data entry and word processing.

Required Qualifications

· Minimum education: LVN (Licensed Vocational Nurse),

· At least 2 years of experience in a community health setting in a healthcare role.

· At least 2 years of experience in community health, clinical, hospital, acute care, direct care, or case management settings.

· At least 2 years of experience with MS Word, Excel, and Outlook.

· Ability to travel within the assigned region to visit Medicaid members in their homes or other settings, including community centers and hospitals.

· Reliable transportation with a valid driver’s license and a good driving record.

· Fluent in Spanish (required).

Preferred Qualifications

· Bachelor’s Degree.

· Experience with electronic charting.

· Experience arranging community resources.

· Field-based work experience.

· Behavioral health experience.

· Background in managing populations with complex medical or behavioral needs.

General Comments

· Travelers only (must have a Texas-specific LVN license; can live out of state. Multistate status cannot be through a different state).

· Location: El Paso, TX.

· City of El Paso Department of Public Health.

· 2-month contract (end date: Friday, May 30, 2025).

· Start date: April 7, 2025.

· Mileage reimbursement provided at $0.70 per mile (daily mileage tracking required).

Spanish Fluency Testing

· Spanish fluency test

Responsibilities

· Scheduling and conducting face-to-face and/or telephonic member assessments.

· Coordinating member care based on needs identified during assessments.

· Completing all required paperwork within state-specific turnaround times.

· Collaborating with members and their LAR to develop individualized care plans.

· Completing comprehensive daily/weekly status and handoff reports.

Software Skills Required

· Proficiency in Microsoft products, Teams, Outlook, and the ability to quickly learn other systems (e.g., OICS).

Hours

· 8 am – 5 pm MST. Training hours will be the same; training days TBD.

Additional Health Screenings

· No additional health screenings required besides a drug and background check.

Additional Considerations

· Contractor MUST be fluent in Spanish (read, write, and speak at a high level). Proof of fluency requested.

· Experience with home health, case management (including hospice), elderly population, mental health, behavioral health, and IDD population (Intellectual or Developmental Disabilities).

· 40 hours guaranteed.

Job Details

· Will ask 134 Health Risk Assessment questions.

· Provided equipment: laptop, monitor, printer, cell phone, hot spot (must be returned after contract ends).

· Minimum of 5 visits per week (push for 7-10 per week).

· Each assessment typically takes 1 to 1.5 hours.

· Pre-work to post-work for one assessment will take approximately 4 hours.

Compliance Requirements Post-Offer

· Car insurance required (copy needed).

· Full backgrounds including DMV report (clean driving record required).

Compensation

· Travel Pay: $15/hr x 40 hrs = $600/week.

· Meals and Food Allowance: $336/week + $770/week = $1,106/week.

· Weekly Total: $1,706/week.
