This is a full-time day position Monday thru Friday 8:00-4:30pm-Must be bilingual

Job Summary:

The CHW uses their expertise about the community they are working with to provide education about chronic disease prevention and treatment.

Essential Functions:

• Engage in community outreach and education about chronic disease screening and health
• Learn and apply clinic policies and procedures to Care teamwork
• Educate community about chronic disease prevention and management
• Engage in relationship-building discussion with patients
• Log enrollments, educational activities, and referrals
• Track and follow-up with patients as part of clinical workflows, supporting team-based care
