- Create a positive experience, Be proactively engaged to the member to manage their care
- Provide member education
- Support transitions of care
- Help to keep members compliant with their care plans
- Liase with care team (community, providers, internal staff)
- Knowledge and continued learning of community cultures and values
- Comprehensive care management & Care coordination
- Encourage/Health promotion
- Individual and family support
- Referral to relevant community and social support services
- Improve health and self - management of conditions
- Provide transition and personalized care to help
- Reduce the progress of chronic disease
- Reduce inappropriate emergency department utilization, preventable hospital re - admissions
- Required Qualifications
- Bachelor's Degree (or higher) in social work, psychology, social services, human services, or NursingĀ Assistant Certified current licensure in the state of WA or National Certification or High School Diploma / GED (or higher) with either an Indian Health Service (IHS), an LPN license in the state of WA or equivalent work experience as a Community Health Worker (CHW)
- Must have resided in King county and within the local community for 1+ years
- Knowledge of culture and values in the local community.
- Familiarity with the resources available within the community.
- Access to reliable transportation that will enable you to travel to client and / or patient sites within a designated area.
- Flexibility, quick learner, ability to be directed and follow instructions.
- Ability to create, copy, edit, send, and save documents, spreadsheets, correspondence in Microsoft Word, Microsoft Excel and Microsoft Outlook.
Employees are expected to wear a mask in areas where physical distancing cannot be attained at ALL times. Preferred Qualifications
- Good client-based communication experience
- Bilingual and Multi-lingual a plus
- Knowledge of WA Medicaid and Medicare populations
- Experience in Counseling or Behavioral Health
- Case management or transitions of care experience preferred.
- Experience working with members of all ages (children to geriatric)
