Amergis LVN Case Manager - La Grange in Georgetown, Texas
The LPN/LVN Referral Specialist is responsible for authorizations, referrals, and case management went on FMLA. The role is being transitioned into a clinical support specialist. LVN Referral Specialist/Case Manager is also responsible for coordinating continuum of care activities for assigned patients and ensuring optimum utilization of resources, service delivery, and compliance with medical regime. Minimum Requirements:
Current LPN/LVN licensure in state practicing
At least one year of Case Management experience preferred
Current CPR if applicable
TB questionnaire, PPD or chest x-ray if applicable
Current Health certificate (per contract or state regulation)
Must meet all federal, state and local requirements
Must be at least 18 years of age
Position Needed:
1 LVN for a case management role to help coordinate care for the primary care clinic.
Responsibilities:
Authorizations and referrals for the La Grange clinic and Giddings clinic.
Primarily phone-based communication with patients.
Candidate should be open to training for and providing bedside care, vital signs, venipuncture, etc. when needed.
Experience: 1 year of experience in case management
Qualifications:
Comfortable talking to patients on the phone.
Interest in serving rural populations.
Must be open to being trained/doing bedside care when need be. They will work out in the nurse's station.
Bilingual in Spanish preferred
The client is open to CMA candidates with a background in case management and coordinating continuing care
Location:
- La Grange Clinic - Primary Care Clinic
Schedule:
Monday to Friday, 8 AM - 5 PM.
