University of Michigan Health-West Referral/Transfer Center Coordinator (RN) in Grand Rapids, Michigan

University of Michigan Health-West (Formally Metro Health) is looking for a Referral/Transfer Center Coordinator (RN).

The referral center coordinator is responsible for managing and coordinating patient transfers to the UMHW. This role ensures seamless, efficient, and safe transfers by working closely with medical staff, transport services, and external healthcare organizations. The coordinator plays a critical role in optimizing patient flow and improving the overall patient and employee care experience. This position ensures that patients receive timely and appropriate care by liaising between other healthcare services and the hospital.

Requirements:

  • Michigan Registered Professional Nurse license required.
  • Bachelor of Science in Nursing or related field desired or working toward degree
  • Current Basic Life Support (BLS/CPR) provider -- required.
  • Certification in a nursing area is encouraged.
  • Minimum of two years recent acute care hospital RN experience within the past 7 years or experience in a referral/transfer center at an acute care hospital.
  • Cognitive and technical knowledge and the ability to progressively manage care delivery across the continuum of care.
  • Ability to retrieve, communicate and present data and information both verbally and in writing required as is the ability to express or exchange ideas by means of the spoken and written word.
  • Broad understanding of patient care processes and strong clinical nursing assessment skills and critical thinking.
  • Involves discretion and independent action within prescribed limits.
  • Demonstrates the following:
    • Handles multiple tasks concurrently, thinks logically and sequentially and creatively problem solves.
    • Responds quickly and adjusts to change.
    • Works independently, organizes and prioritizes work to meet all deadlines.
    • Excellent verbal and written communication skills using SBAR, relationship building, negotiation, persuasion, marketing, and advocacy.

Essential Functions & Responsibilities:

I. Clinical

  1. Coordinates and prioritizes incoming referrals efficiently, ensuring all necessary documentation is complete and accurate.
  2. Accurately enter and update the patient information in the electronic medical record.
  3. Identifies attending physician for patients being admitted. Set up conference call with appropriate attending physicians, consulting physicians, and other individuals as necessary. Communicate incoming transfers with all appropriate departments. Assist with special requests such as transportation, additional testing, image transfer, etc.
  4. Conduct clinical reviews of patient charts to determine appropriate level of service for patient admissions.
  5. Serve as the primary contact for referring providers, patients, and specialists.
  6. Serve as a medical records and physician contact for all patient deaths. Responsible for assuring all appropriate paperwork has been completed and sent to medical records and Risk Management is contacted when appropriate.

II. Personnel

  1. Promotes communication with all patient units, departments, and medical staff, exemplifying high customer service standards.
  2. Accurately track and report referral data and outcomes, ensuring all relevant data is documented.
  3. Works collaboratively with Patient Registration, Emergency Department, Inpatient and Ambulatory areas, and all referring organizations to ensure all services are coordinated effectively.
