Healthfirst Diabetes Health Educator in Hybrid, New York
Primary Responsibilities
Assess community health needs and recommend / prioritize educational programs
Support in the development and execution of health educational programs
Support the creation of objectives and goals
Work with internal team to develop relevant content / materials and facilitate work shops, training sessions and community events
Work with team lead and project associate to track identified metrics and monitor progress
Program effectiveness
Work with clinical and care management team to develop culturally sensitive and competent educational materials and resources
Work to promote healthy behaviors in the community (based on area of focus)
Essential Skills
High school diploma or GED Equivalent
Certificate in Health education (Certified Health Education Specialists) or BA Public Health, Health Education or Related Field
Bi-lingual - Spanish
Should be a community health worker
Experience working with diverse communities, including faith-based organizations and Local healthcare providers
Strong communication, interpersonal and presentation skills
Knowledge of health education principles, practices and cultural competency
Program planning and evaluation
