Summary Geriatrics & Extended Care (G&EC) provides care and counseling of older adults to promote independence, support quality of life and mitigate common syndromes associated with ageing or in the promotion and maintenance of health and prevention of illness and injury based upon the nursing process which includes systematic data gathering, assessment, appropriate nursing judgment, and evaluation of human responses to actual or potential health problems, health teaching, & health counseling. Responsibilities Major Duties: The Clinical Associate Licensed Practical Nurse (LPN/TCT), Geriatrics and Extended Care Service, duties will include, but are not limited to, utilize methods of virtual care such as Home Telehealth (HT), Clinical Video Telehealth (CVT), Secure Messaging (SM), telephone contact. Must demonstrate an exceptional degree of practical nursing knowledge and the skills necessary for concentrated effort and self-direction in carrying out the most complex practical nursing care assignments within the scope of their education and licensure. In addition, employees at this grade level exhibit independent technical judgment, knowledge in leadership, team building, and critical thinking skills, assist complex veterans under the care of multiple providers. Prepares and administers medications. Manages communication with patients through phone calls, secure messaging and/or CPRS alerts. The incumbent will perform diverse LPN/TCT functions such as coordination of patient care, physical assessment, complete geriatric related care reminders, and very significant case management duties. Physical Requirements: Must be able to perform work that requires frequent walking, bending, lifting of 45 pounds or more, stooping, reaching, stretching, and similar activities supporting various weights. Other activities include pushing carts, equipment, and wheelchairs. Must be able to care for patients with medical and psychiatric illnesses and must be able to care for patients who have a potential to be physically and verbally abusive. Must have adequate hearing to be able to respond to various interactions over telecommunication systems and must have good verbal and written communication skills.
