Intandem Children's Health Home Care Manager in Jamestown, New York
JOB TITLE: Childrens Health Home Care Manager CLASSIFICATION: Full Time, Non-Exempt SALARY: $21.84 per hour starting rate POSITION SUMMARY: Works with people with qualifying Physical/Mental Health/Substance Abuse/Developmental Disability diagnoses and their natural supports to link/monitor/refer to services that meet their individual needs and personal goals. Also provides ongoing advocacy. Provides ongoing advocacy through person centered approach. Meets documentation requirements of regulating organizations by completing documentation. QUALIFICATIONS: Minimum Education, Training & Experience: Associates' Degree in a Human Services related field. One to Two (1-2) years of relevant experience in Human services (including but not limited to Mental Health, Social Work, Substance Use, Education, etc.)., with a minimum of 6-12 months experience developing Care Plans/SMART Goals and functional knowledge of community resources. **For applicants with an Associates degree, a waiver is required to be completed and accepted by The New York State Department of Health before hire.
Preferred Education, Training & Experience: Bachelors Degree of Arts or Science Two (2) years of relevant experience in Human services (including but not limited to Mental Health, Social Work, Substance Use, Education, etc.), with a minimum of 6-12 months experience developing Care Plans/SMART Goals and functional knowledge of community resources. **Applicants with a Bachelors Degree with less than 2 years experience may be considered, but a waiver is required to be completed and accepted by The New York State Department of Health before hire
ESSENTIAL SKILLS AND ABILITIES: Valid New York State drivers license and dependable transportation that meets the Agency standards Ability to work flexible hours and travel throughout Counties Served: Allegany, Cattaraugus, and Chautauqua (Caseloads may cover two adjacent counties) Ability to work effectively and independently including address multiple tasks efficiently Strong organizational and analytical skills Ability to think creatively and be results oriented Ability to effectively communicate verbally and in writing including developing rapport with the people we serve.
