The Medical Social Worker is a member of the Interdisciplinary Health team and is responsible
for providing social work services in the area of Hospice care. The social worker may provide
direct care, teach, supervise and consult.
Visits hospice patients and families to complete psychosocial assessment provide education and support.,
a. Assists patients and families in resolving personal and environmental difficulties, by providing information, referral, coordination and advocacy as needed to
enhance their ability to cope with factors related to their illness.
b. Counsels and assists patients and their families to participate in the development of the hospice plan of care.
Serves as supportive caregiver to the Hospice Interdisciplinary Team.
Responsible for the development, implementation and timely revision of the care plan in coordination with the family and hospice team.
a. Participates in interdisciplinary team meetings providing Psychosocial assessment information relevant to the plan of care.
b. Assesses, documents and reports to the hospice interdisciplinary team the social and emotional factors concerning patient’s capacity and potential to cope with problems of daily living
Accurately completes all documents pertaining to patient care and services provided within 24 hours in a neat, timely, accurate, concise and legible records and reports.
Other duties as required, including coordination and assignment of social service referrals, participation in the agency’s Ethics Committee and Crisis Intervention Team as appropriate.
Is available for emergency on-call service.
ADDITIONAL REQUIREMENTS:
Possession of a current license to operate a motor vehicle
Must have access to car for agency business purposes.

Very few medium occupations in the national economy are performed primarily in a sitting position
