• Care Coordinators working for SeniorsPlus in the State of Maine assist clients to make informed choices to live safely and independently in their communities.
• Support is provided primarily through telephonic care coordination, assisting clients to achieve maximum independence and to positively impact and improve their well-being.
• All of this occurs through a collaborative process of assessment, planning, implementing, and monitoring with other health care professionals to coordinate the clients plan of care.
• Some travel and home visits will be required.

EDUCATION and/or EXPERIENCE

• Bachelors degree in Health or Social Services required
• Minimum of one year in Social/Human/Community Services required
• Familiarity with the long term care system a plus, strong computer and organizational skills required
