The position will be supporting the Area Agencies on Aging (AAA) in Barren River. Responsibilities include bridging the gap between clinical care and community services, ensuring that older adults receive comprehensive and patient-centered care, and utilizing data collection skills to ensure seamless collection of outcomes. Responsibilities include ensuring programs align with the latest evidence-based practices and meet the specific needs of individuals with Alzheimer's and related dementias (ADRD), delivering training on dementia care for healthcare and supportive care workers, leveraging resources to enhance skills and understanding of ADRD. This individual will also be responsible for guiding organizations to become more age-friendly when it comes to providing care for adults experiencing ADRD.The position is grant funded and are contingent upon the renewal of those funds.
