M Health Fairview has an immediate opening for an RN Case Manager to join our team at the Vascular Clinic. Join MHealth Fairview, the largest healthcare system in the state! The Vascular Clinic is a growing and collaborative clinic. Our clinic hours are 8AM to 4:30PM Monday - Friday, so no evenings, weekends, or holidays! This provides an excellent work life balance!
This is a 0.8 FTE (64 hours per two week pay period) opening.

Responsibilities Job Description
The Vascular Center Case Manager RN works in a variety of different roles and clinics in the department. Case management of Vascular, Vein and Wound patients ensuring all the needs and educate of the patient are needed. Performs triaging on incoming nursing calls and questions. Independent Wound and Education Visits with patients.
Team collaboration between Supervising Physician, LPN’s, CMA’s and Patients.
A mix between Vascular surgery case management, triage, Wound care, Vein procedures and Podiatry. The ability to go to 2 sites St Johns and woodwinds with the possibility of Wyoming Lakes hospital on occassion.
Completes non-invasive testing procedures on patients in HEVC as directed.
Under the direction of the Vascular Center physician/nurse practitioner, prepares and implements acare plan according to the Vascular Center written orders and updates appropriately.
Provides ancillary patient care needs to include, but not limited to: review of laboratory and culturedata; appropriate interaction with physicians and pharmacies; triaging of telephone calls; interactionwith caregivers.
Completes and documents wound and vascular assessment on both new and follow-up patients foreach patient visit.
Assess patient's educational needs.
Provides communication between the Center and ancillary departments and care givers to facilitatecompliance with vascular and wound plans of care including orthotics, physical medicine, etc.
Implements wound and vascular patient care protocols where positive viral screens occur.
Coordinates the ordering of clinical supplies and PDWHF as needed.
Maintains knowledge of, and complies with, all relevant laws, regulations, policies, procedures andstandards.
Actively participates in creating and implementing improvements.
Performs other responsibilities as needed/assigned.

Associate's Degree in nursing
License/Certification/Registration
Basic Life Support
Registered Nurse MN
Preferred
Education
Bachelor’s Degree Preferred
Experience
1-2 years prior RN experience preferred
Wound Care experience preferred
Acute Care experience preferred
License/Certification/Registration
WOC preferred
Vascular Certification preferred
