Job Purpose: Connects and supports families referred to the Next Step Clinic and assists families during and after the clinical testing process. Conducts initial screenings for children's developmental or mental health challenges, develops family care plans to assess household needs and any potential barriers to care, and advocates and assists families in navigating the mental and developmental service systems of Milwaukee. Provides community outreach and represents the clinic through community partnerships, resource fairs, and community meetings.

Essential Job Functions:

• Contacts referred families to Next Step Clinic.

• Screens child for ASD risk.

• Supports families in completing registration paperwork.

• Schedules for families for evaluation and reschedules as needed.

• Serves as a source of support and information during and after the testing process.

• Supports families in rescheduling and helping them through obstacles.

• Attends the beginning of the first session of evaluation (handoff to evaluation team).

• Attends findings meeting with all evaluation families.

• Supports families navigating services after the evaluation is complete.

• Available to support groups for parents.

• Attends resource fairs.

• Maintains community partnerships through regular communication.

• Conducts outreach into the community to raise awareness and connect families to the Clinic.

• Represents Clinic at community meetings and events.

• Recruits Community Advisory Board members and leads Community Advisory Board meetings.

• Conducts family navigation for non-evaluation families.

• Other duties as assigned.

Qualifications

Required Education, Experience, Certifications, Licensure, and Credentials : (Where appropriate, education and/or experience may be substituted)

Minimum Required Education : High School Diploma.

Preferred a bachelor’s degree in social work, Public Health, or Health Care Administration.

Minimum Required Experience : Preferred experience working in healthcare or with children with autism and their families.

Required License – Certification – Registration : Issued by Governing Body (if applicable): N/A

Travel Type: Up to 50%

Required Valid Driver’s License: Valid Wisconsin driver’s license.

Required Auto Insurance: Vehicle liability insurance in accordance with Agency policy.

Knowledge, Skills, & Abilities :

• Effective oral and written communication, time management, and mathematical skills.

• Ability to effectively communicate clearly and concisely in both oral and written form.

• Knowledgeable of EBP for autism and trauma in children and adults.

• Competent in the administration of screenings, assessments, and documentation for children.

• Knowledge of the service continuum and network for children and service providers in Milwaukee County.

• Knowledge of using work-related tools such as computers, cell phones, texting, photocopiers, and Microsoft Office applications such as Teams and Outlook.

Physical Requirements, Visual Acuity, and Work Conditions:

Physical Requirements : On occasion must be able to push/pull/lift/move objects weighing at least 20 pounds. Must have the ability to climb at least two flights of stairs. Work is performed largely in the community but may at times include an office environment or other various settings.

Visual Acuity: The worker is required to have close visual acuity to perform activities such as preparing and analyzing data and figures, transcribing, and viewing a computer terminal.

Working Conditions : Ability to successfully deal with change and potentially stressful situations.

This position is in an office environment and requires the occasional use of stairs. Distinguish when non-communicative consumers need support and converse clearly with consumers and family members frequently.
