Position Summary: The Community Health Resource department works with individuals who are experiencing barriers to accessing health care services. The Community Health Worker establishes positive relationships with participants and provides feedback that reinforces use of primary care providers as the participant’s point of care, and assistance with barriers to accessing or adhering to care plans as well as build community partnership that support individual engagement in age and gender preventative services. Adherence to HIPPA Policies always.

Responsibilities Include:

Community Outreach

Build bridges between participants and the Health Education Services available by developing and participation in community outreach activities. Build and maintain positive working relationships with hospital partners, community members, medical providers, community agencies, from diverse backgrounds to promote the services offered by Community Health Resources and engagement with recommended age and gender preventative care. These services include but are not limited to Diabetes Prevention Program, diabetes education, Hypertension Program, gym scholarships with Health Point and the YMCA partnerships, and preventative screenings such as mammograms and colonoscopies.

Multi-Disciplinary Team Participation

As members of a multi-disciplinary team Community Health Workers collaborate with team members to complete tasks and duties in supporting the work of the Community Health Resources department. Participation in case staffing is required to effectively communicate with team members and meet the need of the participants in a patient centered manner. The Community Health Worker will directly support and participate in the department’s wellness events.

Community Collaborations

Participate in community collaboratives as assigned. The role of the Community Health Worker in the collaborative will be to increase Tidelands Health presence in various sectors of the community, increase awareness of Tidelands Health resources, transferring knowledge of a collaboratives work to the broader staff in the Community Health Resource team. Attendance at meetings related to the collaborative are required as assigned.

Other Responsibilities

  • Document using the approved software in a timely and effective manner adhering to policies and procedures related to HIPAA

  • The community health worker always exhibits exemplary customer relationships skills

  • Completes other duties as needed

  • Adhere to strength focused language for conversations related to participant behaviors and care planning needs Will be required to transport participants to healthcare appointments, as needed, utilizing TCCN departmental vehicle

QUALIFICATIONS

Education:

  • High school diploma or equivalent, required.

  • Bachelors or Associate Degree, preferred.

Experience/Qualifications:

  • Any combination of three (3) years health/social services experience

  • Bilingual speakers, specifically Spanish, highly encouraged to apply

Special Skills:

  • Proficient communication skills, both oral and written, required.

  • Ability to Multi-task

  • Proficient in using word, excel, and outlook.

Other:

  • Valid and in good standing South Carolina driver’s license, required.

  • Ability to work weekends and attend evening events, as needed, required.

  • Familiarity with the rural communities in Georgetown and Horry Counties area, required.

Physical Requirements: Light-Medium Physical Agility Test Rating (PAT)

While performing the duties of this job, the employee is frequently (activity or condition exists from 1/3 to 2/3 of the time) required to stand, sit, and walk; occasionally (activity or condition exists up to 1/3 of the time) to use hands, fingers; and frequently to talk or hear. The employee must exert 15 to 20 pounds of force occasionally, and/or 5 to 10 pounds of force frequently, and/or greater than negligible up to 5 pounds of force constantly to move objects.

The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job and will be measured by post offer PAT.

Works in a variety of settings throughout the community requiring travel from one destination to another using personal transportation. Days and hours of work are set by the department director based on the needs of the system and are subject to change at any time but position exercises considerable independent judgment and decision making skills and operate in an exempt fashion.
