Tidelands Health Community Outreach Nurse (LPN) in Murrells Inlet, South Carolina

Position Summary :

Community Outreach Nurse will be responsible for:

  • Program development and leadership for clinical and community outreach services in the community health resources department.

  • Responsible for leading the planning, managing, staffing, and implementing community health screening events, community health fairs, clinical/community screenings at our partner locations (YMCA, libraries, community events, etc.), mobile health outreach, and other outreach events that represent the hospital system.

  • Responsible for development and training of staffing for these events and outreach

  • Responsible for management of staff at events and outreach, as well as recruiting, onboarding and retaining staff on an on-going basis based on needs and community events/outreach calendar and program development.

  • Responsible for making and maintaining community partnerships and relationships and representing Tidelands Health

  • Responsible for providing health education and chronic disease management services to target populations

  • Ensuring policies, procedures, and standard work are developed and reviewed annually for all aspects of community outreach programming under this position’s oversight.

  • Ensuring competencies, equipment and supplies needed for screenings are calibrated, tested, operational and adequate to maintain regulatory guidelines.

  • Responsible for teaching classes on a variety of topics and for different audiences, as needed

  • Responsible for ensuring the completion of ongoing data collection to support the CHNA and CBR and ensuring identified needs are considered in the development of the strategic direction for the organization’s wellness initiatives.

  • Provide assistance as needed to the overall Community Health Resources department and, in general, complete other duties as assigned.

QUALIFICATIONS

Education :

• Five (5) or more years of experience as a hospital/clinical-based LPN, required.

• Previous experience in healthcare education setting, preferred.

• Prior experience in community health education and instruction, preferred.

Experience :

• Completion of an accredited postsecondary LPN certificate program required.

License/Certification :

• LPN Licensure with South Carolina Board of Nursing required.

• American Heart Association Healthcare provider course with Basic Life Support (BLS) certification required to be initially completed with the TH Organizational Development Office within 30 calendar days of hire date (exceptions to obtain certification from a provider other than the TH Organizational Development Office are approved by TH Organizational Development on a case by case basis). BLS Recertification is required every two (2) years.

• BLS instructor certification required within one (1) year of hire date.

• Certified Diabetes Certification, preferred.

• Safe Sitter certification preferred upon hire into position (or willingness to work toward certification in this area).

Knowledge/Skills/Abilities :

  • Working knowledge of the healthcare system, population health and knowledge of the social determinants of health.

  • Ability to work independently, adjust priorities often and establish rapport quickly.

  • Demonstrated experience in diplomacy and ability to contribute to team goal setting.

  • Strong interpersonal and communication skills.

  • Plan: determine strategies and assist with plans for community wellness events and activities.

  • Build Relationships: establish and maintain positive working relationships with others both internally and externally to achieve the goals of the coalition.

  • Organization: set priorities, develop a work schedule, monitor progress towards goals, and track details/data/information/activities.

  • Proficient PC skills and experience of Windows and Microsoft office programs required.

Other :

• Personal transportation to conduct services and transport materials and supplies to service location throughout the region, required.

• Valid SC driver’s license in good standing, required.
