Job Summary and Qualifications

POSITION SUMMARY: Provides patient with care that is continuously evaluated, planned for, and revised as needed. Documents the patient status with assessments, reassessments, patient care needs, and interventions to meet the patient’s needs. Documents nursing care provided, the patient’s response to, and the outcome of care provided. Organizes and prioritizes work to provide health information, discharge planning, and patient care in a safe environment to maximize health in the continuum of care and protect the patient. Assumes responsibility for an assigned group of patients and be accountable for an assessment of the patient’s basic human needs, enabling team of providers to make appropriate decisions to develop, implement and evaluate a plan for care, to include physical needs, bio-psychological needs, environmental/safety needs, self care needs, educational needs, and discharge planning needs. Professional and competent as a care provider, a leader of personnel, and a collaborator with other professionals. Demonstrates awareness of his/her unique position in the Department and responsibilities as a professional R.N. through accurate, clear, concise and timely written and oral communications.

POSITION QUALIFICATIONS

LICENSURE/CERTIFICATION:

  • Maintains a current Utah Registered Nurse (RN) license. Compact license accepted.

  • American Red Cross or American Heart Association Basic Life Support Course (BLS or BCLS) certification within 7 days of hire

EDUCATION/ QUALIFICATIONS REQUIRED:

  • Graduate of an accredited school of professional nursing. Proficient.

  • 1 or more Years RN Exp

EXPERIENCE/ JOB KNOWLEDGE REQUIRED: Ability to practice nursing as described by the following definition: The diagnosis and treatment of human responses to actual or potential health alterations. Ability to utilize the nursing process in the provision of nursing care including but not limited to administering treatments and medication, interpreting diagnostic tests, formulating a plan of care, collaborating with other health care providers and providing education to patients and/or significant others. Math computational skills of dosage calculations. The successful candidate will be required to pass competencies for this position.

Physical Requirements: The position requires a considerable amount of physical work not to exceed 50 pounds. Any workload exceeding 50 pounds will require assistance. The individual will be required to pull up or reposition patients in bed, lift supplies, place equipment on supports push beds throughout the hospital, and perform external cardiac compressions. The individual must be able to bend over to perform certain duties, e.g., wound dressing removal, etc. The individual must be able to spend the majority of the shift in a standing position and must be able to maneuver throughout the halls, and patient’s rooms in response to hospital emergencies. Interpretation of environmental input requires visual and auditory skills. In the event there is a need to evacuate the building, heavy lifting will be required to carry patients to safety. A significant amount of writing and computer use is required. Trips to various departments will be required.

Working Conditions: The individual spends over 95% of her/ his time in an air-conditioned environment with varying exposure to noise. There is protection from weather conditions but not necessarily from temperature changes. The position does have heavy exposure to malodorous, infections body fluids from patients and some minimal exposure to noxious smells from cleansing agents.

Personal Protective Equipment (PPE): This position requires high exposure to infections wastes such as blood and body fluids that mandate the wearing of gloves, masks, gowns and goggles for every actual or potential exposure

Direct Patient Care Age Population: Demonstrates knowledge of the principles of growth and development and possesses the ability to respond to specific issues and data reflective of the patients status. Demonstrates the knowledge and skills necessary to provide for the primary population of patients served in the department. Demonstrates ability to adapt to varied age-specific patient populations. All age groups apply.

MAJOR TASKS, DUTIES, AND RESPONSIBILITIES:

· Provides direct care for patients in the clinic setting including holistic assessment, comprehensive wound evaluations according to protocols, and preparation of patient for medical appropriately.

· Integrates diagnosis data into health assessment notifying physicians of results in a timely fashion and documenting appropriately.

· Uses knowledge and understanding of algorithms for wound care.

· Exercises clinical competence in nursing skills and follows procedures based on WCC, and hospital based policies and procedures.

· Administers pharmaceutical and blood products as ordered.

· Demonstrated competence in ordered treatments including but not limited to vital signs, capillary blood sugars, vascular assessments, wound assessments, sterile technique, dressings, phlebotomy, handling of autologous blood products, and sterile syringe fill techniques.

· Demonstrates competence in patient and family education on common topics including but not limited to wound care, dressing applications, compression therapy, diabetic foot care, venous life style changes, nutrition, and wound off-loading.

· Adheres to guidelines or standards of practice (i.e. ANA), community, and associate hospital, AMD, all current WCC policies and procedures.

· Safety precautions are implemented as needed by professional judgment/policy and procedures.

· Demonstrates consistent use of OSHA guidelines and Universal Precautions.

· Communicable diseases are reported per policy.

· Contributes to the professional development of peers, colleagues, and others.

· Decisions and action on behalf of patients are determined in an ethical manner.

· Collaborates with the patient, significant others, and health care providers in providing patient care.

· Demonstrates maturity and accountability for job performance. Supports the philosophy, objectives and goals of the Department and assesses areas of personal and professional growth.

· Supports and promotes excellence in customer / guest relations.

· Supports and promotes the organizations Infection Control, Safety, and Risk Management Programs.

· Provides holistic nursing care to meet physical, emotional, spiritual, educational and socio-cultural needs of the patient/ family.

· Performs routine IV therapy.

· Operates and monitors specialized equipment such as cardiac monitors, nasogastric suction, pulse oximeters, infusion pumps, non-invasive blood pressure monitoring equipment, etc.

· Provides specialized treatments and procedures such as: IV monitoring, compression therapy, and specimen collection.

· Reports and documents in the health record via hospital accepted format including; observations, facts, assessments, plan of care, care given and outcomes of the patients care; patient classification and acuity; and other data relevant to the patient care in an appropriate and timely manner.

· Accepts physician orders verbally or via telephone. Transcribes physician orders; prepares and administers prescribed medications and treatments according to nursing and pharmacy policy and procedure.

· Recognizes the need for and initiates emergency measures such as cardiopulmonary resuscitation. Notifies the appropriate health care provider in a timely manner.

· Participates in nursing care conferences.

  • Complies with and is knowledgeable of hospital and department safety procedures and polices.

  • Contributes to the care of the environment by maintaining work areas and assisting with light housekeeping tasks as needed.

  • Is knowledgeable of and participates in the hospitals performance improvement program.

  • Utilizes computer system as needed for information and to complete tasks.

  • Maintains customers rights (confidentiality, privacy, safety, security, and decision making)

  • Interacts with customers, who may include patients, patient’s families, physicians, co-workers, and visitors, in a manner that exemplifies the hospitals mission and core values.

  • Attends scheduled in-service, department staff meetings (10 per year), annual skills update, and other training sessions relative to the job.

  • Is knowledgeable of and adheres to hospital and department procedures and policies

  • Adapts easily to changes in work assignment and environment.

  • Is willing to assume additional responsibilities.

  • Performs basic tasks in a timely manner with minimum guidance and supervision.

  • Demonstrates willingness to work toward department and hospital goals and objectives.
