Amergis Healthcare is currently hiring a Case Manager/Social Worker to help support an Adult Day Health Center!
Social Worker/Case Manager Job Description:
Responsibilities:
Provide an Initial Assessment, Quarterly Assessment & Reauthorization Assessment. Establish an Individualized Plan of Care (“IPC”). The I PC shall be based on the SW Assessment and should include a Problem, an Intervention, and a Goal. Act as a member of the Multidisciplinary Team. Provide in-home assessments for facility prior to admission. Provide signed and dated documentation for all the services performed the day the services are provided and include signed and dated progress reports in each participant record.
Will be required to drive their own vehicle to client intake assessments. This will be within a 15 mi radius and may occur 3-4 times a month.
Job Description:
Location: Irvine, CA
Schedule: M – F, 8:30 AM to 2 PM
Setting: Adult Day Health Center
Pay: $35/hour – dependent on experience
Duration: 13 weeks (with possibility of extension!
Qualifications:
Holds a master’s degree in social work from an accredited school of social work.
Holds a master’s degree in psychology, gerontology, or counseling from an accredited school and has at least 6 months to one year of experience providing social services in aging, health, or long-term care services.
Is licensed by the California Board of Behavioral Sciences.
Holds a bachelor’s degree in social work from an accredited school with at least 6 months to a year of experience providing social services in aging, health, or long-term care services.
