RN Case Manager

Job Description

The RN Case Manager performs telephonic case management for Medicare Advantage members in the state of Florida. The case manager will be responsible for performing care coordination activities, developing care plans and assessments, conducting new referrals, calling members, and building new cases in Athena (EMR).

Responsibilities

• Perform telephonic case management for Medicare Advantage members.

• Conduct care coordination activities.

• Develop care plans and assessments.

• Handle new referrals and call members.

• Build new cases in Athena (EMR).

Essential Skills

• RN license (FL license required)

• 2 years of case management experience from hospital/acute care/home health/hospice

• Experience with Athena EMR software/platform

• Strong computer skills: Microsoft Suites, Excel, data entry, and computer documentation

• Ability to handle a caseload of at least 73

• Patient care

• Triage

• Medical record management

• Customer service

• Health care

• Utilization management

Additional Skills & Qualifications

• Prior experience in a fast-paced/high acuity nursing environment

Work Environment

This is a hybrid opportunity with working hours from Monday to Friday, 8am to 5pm. Candidates must have a genuine desire to work in case management because they enjoy the work itself.

Pay and Benefits

The pay range for this position is $35.00 - $45.00/hr.
