Position Title: Community Empowerment Navigator (Case Manager)

Position Overview
The Community Empowerment Navigator (Case Manager) is a cornerstone of Sisters PGH's mission to provide life-saving and transformative services to transgender and nonbinary individuals in Pittsburgh. This fully in-person role is critical in delivering trauma-informed care, building trust within the community, and creating pathways for empowerment and self-sufficiency.
The Community Empowerment Navigator will work closely with the Trans Resource Center Office Administrator, Project T Coordinator, Behavioral Health Program Manager, and Executive Director to ensure clients receive comprehensive, wraparound support tailored to their needs. While the Navigator focuses on resource navigation, case management, and external service connections, they will collaborate with the Behavioral Health Program Manager as a referral source for clients requiring mental health services such as counseling and therapeutic support. Together, these roles form a seamless continuum of care with clearly defined, yet complementary, responsibilities.
This hourly, part-time position pays $40.38 per hour (equivalent to $42,000 annually, based on 20 hours per week). There is potential for this role to transition into a full-time position with expanded responsibilities and benefits.
Key Responsibilities
Client Services and Case Management
Conduct comprehensive assessments to identify immediate and long-term needs.
Develop, implement, and monitor individualized service plans that focus on empowerment and self-sufficiency.
Provide trauma-informed support, crisis intervention, and emotional assistance to clients.
Connect clients with internal and external resources, including housing, healthcare, and employment support.
Collaborate with the Behavioral Health Program Manager to ensure clients with mental health needs are referred for counseling and therapeutic services.
Maintain ongoing follow-ups with clients to monitor progress and adjust service plans as needed.
Travel to meet clients in their homes, shelters, or other locations when in-office services are not feasible.
Collaboration and Coordination
1. Work closely with the Trans Resource Center Office Administrator, Project T Coordinator, Behavioral Health Program Manager, and Executive Director to deliver cohesive and effective support to clients.
2. Partner with the Behavioral Health Program Manager as a referral source for clients requiring mental health services, ensuring seamless care.
3. Develop and maintain relationships with external community organizations and service providers to expand resource access.
4. Represent Sisters PGH at meetings, conferences, and advocacy events as required.
Documentation and Reporting
1. Maintain detailed, accurate client records, including service plans and case notes, in compliance with organizational policies.
2. Compile data and contribute to monthly and annual program reports to demonstrate impact.
3. Ensure confidentiality is upheld in all client interactions and documentation.
Qualifications
Preferred Education and Experience
• A bachelor's degree in social work, psychology, human services, or a related field is a plus but not required.
• Relevant professional or lived experience working with LGBTQIA communities, particularly trans and nonbinary individuals, will be considered.
• Familiarity with systemic barriers affecting trans and nonbinary individuals is highly valued.
