Center for Human Development Community Transition Team Clinician in Pittsfield, Massachusetts

The primary role of the Clinical Transition Team (CTT) is to ensure continuity of clinical services to youth and families as youth transition from residential placement through a temporary, time-limited support model. CTT clinicians serve as a community-based clinical support who work in concert with the Department of Youth Services (DYS) community casework teams, DYS regional leadership, and Community Services Network (CSN) staff. The CTT Model is aimed at maximizing effective treatment and increased length of stay in the community by providing clinical interventions and support to youth and families through transition to and stabilization in the community, ensuring eventual hand-off to community-based clinical supports or completion of short-term treatment goals. Duties include: - Provides individual and family therapy to identified cases with a focus on risks/needs and ensures continuity of care as youth transition from residential placement. - Provides the primary clinical intervention and support for identified youth once the youth has transitioned to community placement for a time limited intervention and works with the full Clinical Transition Team to connect youth and family to appropriate community services - CTT clinician works with the CSN Family Partner to lead or assist in the facilitation of Family Groups - Full Time, Mon-Fri, 9-5 with occasional evening hours Qualifications: Preferred: Masters of Science or better.
