Robert Half Case Manager in San Francisco, California
Description We are in search of a Case Manager to be part of our team in San Francisco, California. The Case Manager will be entrusted with the task of managing a caseload of single adults who were previously homeless, many of whom are struggling with mental health disabilities, substance abuse issues, and/or medical conditions. This position entails conducting outreach visits, needs assessments, providing a full range of case management services, and assisting with maintaining housing and improving life quality.
Responsibilities:
• Oversee a caseload of single adults, many of whom have mental health disabilities, substance abuse issues, and/or medical issues.
• Carry out outreach visits upon entry to the hotel and as needed thereafter.
• Undertake initial and subsequent needs assessments, providing comprehensive case management services.
• Facilitate housing retention and life quality improvement through effective intervention strategies.
• Assist in establishing and maintaining appropriate benefits and ensuring regular rent payments.
• Address unit habitability issues and accompany hotel managers on pest control and unit inspection visits.
• Refer suitable tenants to job placement programs when necessary.
• Foster a supportive tenant community by coordinating regular community events and social activities.
• Refer tenants to appropriate services based on their needs and support successful connections to these services.
• Participate in agency-wide initiatives and provide support to hotel staff and tenants on monthly check disbursement day.
• Maintain comprehensive documentation and database records of tenant contact and services provided.
• Respond to tenant-related crises, offering de-escalation intervention and support to agitated tenants.
• Collaborate with departments and outside providers to advocate for tenants.
• Ensure confidentiality of tenant/client records and concerns.
• Attend scheduled meetings and participate as requested. Requirements • Proven experience in administrative assistance is essential for this role.
• Proficiency in data entry tasks is required.
