JOB SUMMARY

Responsible for direction of patient care that promotes safety and well-being of all patients in assigned care unit(s) on a twenty-four hour basis. Plans, directs, coordinates and evaluates the overall clinical nursing practice and administrative activities: including fiscal, material and human resource management. Participates in the development and implementation of hospital policies and procedures. Maintains open communication with all staff to promote improvement in performance. This position requires a sound knowledge of nursing practice, exceptional leadership abilities, and solid problem- solving skills. Maintains performance improvement activities within the department and participates in Quality Improvement activities.

ESSENTIAL DUTIES/RESPONSIBILITIES

  • Responsible for creating business plans for the Transitional Care Unit to include all aspects of growing the unit to full capacity and meet occupancy goals.

  • Ability to meet with key stakeholders to improve market share in the region.

  • Responsible for developing, directing, and managing outcomes and multi-year strategies of the Transitional Care Unit.

  • Provides effective leadership; supports and empowers staff to embrace the mission and goals of the hospital.

  • Consults other departments, as appropriate, to collaborate in patient care and provide for an interdisciplinary approach. Maintains a good working relationship both within the department and with other departments.

  • Possesses the specialized knowledge and clinical skills necessary to provide care for patients with physical disabilities and chronic illnesses.

  • Coordinates educational activities and uses appropriate resources to help staff develop and implement individualized care plans for patients and families.

  • Participates in interdisciplinary team conferences collaborating with other healthcare professionals to ensure patients have opportunities for recovery and optimal restoration of function and eventual discharge.

  • Collaborates with team members to achieve cost-effective care by utilizing appropriate clinical measures to meet patients’ physical, psychosocial, cultural, ethnic and spiritual needs of patients and families.

  • Communicates appropriately and clearly to physicians, staff, Director of Nursing Services, Chief Clinical Officer and administrative team.

  • Demonstrates an ability to be flexible, organized and function under stressful situations.

  • Responsible for performance improvement activities for Transitional Care Services. Educates staff on the PI process. Participates in PI data collection, indicator development, and evaluation criteria. Involved in at least one PI activity for each department per year. Maintains appropriate quality control programs.

  • Utilizes LEAN techniques to advance the departmental managing for daily improvement.

  • Performs probationary and annual employee performance evaluation, competency clinical skills assessment and age-specific competency assessment for nursing staff within the Transitional Care Unit department.

  • Keeps employees informed about their work assignment, work progress, and opportunities for advancement.

  • Participates in the development, implementation, review, and revision of departmental policies and procedures.

  • Assures documentation meets current standards and policies.

  • Responsible for interviewing, hiring, training, developing, disciplining, and terminating employees within the departments of oversight.

  • Develops, justifies and maintains a fiscal budget for Transitional Care Unit.

  • Complies with ongoing departmental inspections of equipment and physical space. Makes recommendations regarding equipment utilization and needs.

  • Knowledge of the 5 Rights of Delegation: right task, circumstance, communication, person, supervision, and feedback.

  • Conducts regular staff meetings with unit education for each shift. Maintains documentation of meetings and provides written minutes for absent staff.

  • Conducts all aspects of supervision in a firm, fair, and consistent manner. Gives positive feedback publicly and constructive criticism thoughtfully and privately.

  • Ensures that all rules and regulations are observed by employees, to include overall corporate compliance and compliance with JCAHO standards.

  • Participates in strategic planning for department in line with the organizational strategic goals.

  • Attends a minimum of 75%% of Department Head meetings per year.

  • Demonstrates knowledge of the Five Medication Rights with regard to patient age and condition. Reduces the potential for medication errors.

  • Performs patient care responsibilities, considering needs specific to the standard of care for patient’s age.

  • Demonstrates knowledge of the principles of growth and development over the life span and the skills necessary to provide age appropriate care to the patient population served. Able to interpret data about the patient’s status in order to identify age specific needs and to provide appropriate care.

  • Ability to adequately assess and reassess pain. Utilizes appropriate pain management techniques. Educates the patient and family regarding pain management.

  • Performs all aspects of patient care in an environment that optimizes patient safety and reduces the likelihood of medical/healthcare errors.

  • Functions as a liaison between patient/family and physicians, other healthcare providers, and administration.

  • Formulates a teaching plan based upon identified learning needs and evaluates effectiveness of learning. Includes the patient’s family in the teaching process, as appropriate.

  • Interacts professionally with the patient and their family and involves them in the formulation of the plan of care.

  • Demonstrates adherence to infection control policies. Uses PPE correctly.

  • Demonstrates knowledge of OSHA regulations: complies with Blood borne Pathogen, TB exposure plans, and Hazardous Waste Management Program.

  • Able to perform all age specific competencies for populations served.

  • Able to perform all unit specific competencies of clinical areas worked.

    Supervisory duties and responsibilities include:

  • Writes and conducts annual performance appraisals and ensures regular ongoing feedback, coaching, and communication with staff. Provides development opportunities for staff to broaden and enhance skills and abilities.

  • Ensures high level of performance standards and achievement to meet organization needs and business goals.

  • Administers organization policies and procedures.

  • Ensures compliance with safety policies and good housekeeping.

  • Manages initial training of new employees and ongoing training as required.

  • Promotes an environment that fosters team work and commitment to satisfy customer requirements.

JOB QUALIFICATIONS

Education, Experience, License and Certifications:

  • Current Wyoming Registered Nurse Licensure without restrictions

  • Minimum of 3 years supervisory experience, required.

  • Minimum of 3 years of related experience, preferred.

  • BSN or equivalent experience required, Master's degree in Nursing, Business, Health Administration or related field preferred.

  • BLS certification, required.

  • Knowledge of business planning, preferred.

    Additional Skills:

  • Basic computer knowledge

  • Proficient in the use of Lean management practices.

  • Excellent written and verbal communication; proficient use of English language.

  • Exercises a high degree of initiative, judgment, and discretion.

  • Demonstrates solid abilities to analyze situations accurately and taking effective action.

  • Establishes and maintains effective work relationships establishing and maintaining effective working relationships.

  • Proficient in organizing work, delegating and achieving goals and objectives.

  • Demonstrates high level of judgment and discretion in developing, interpreting, and implementing policies and procedures.

  • Works effectively with physicians, other medical staff, and external agencies. Demonstrates excellent customer relations skills.
