Northeastern Vermont Regional Hospital Care Manager in St. Johnsbury, Vermont

Ready to make an impact in the community? Northeastern Vermont Regional Hospital (NVRH) seeks a Care Manager to join the team. The Care Manager works collaboratively with the Care Management department, interdepartmental healthcare teams, and community partners to ensure continuity of care. The Care Manager is responsible for providing collaborative departmental support to achieve metrics for conditions of participation, including utilization review, medical social work, discharge planning, crisis mental health, and the NVRH Swingbed Program.

Roles and Responsibilities:

  • Assess patients and their family systems to obtain information about their home environment, family relationships, health history, and cultural values. When appropriate, coordinate discussions with patients and/or families to facilitate understanding of medical recommendations.

  • Participate in daily Interdisciplinary Rounds. Communicate with physicians and other clinical personnel in developing and implementing patient-centered discharge plans for inpatients admitted at NVRH.

  • Communicate and coordinate discharge care plans in line with patient wishes with internal clinical personnel and across the continuum of care and serve as a patient advocate.

Qualifications:

Candidates need a Bachelor’s Degree in Human Services, Social Work, or a related field. Master’s in Social Work preferred. If not currently certified, you must be willing to become certified in Case Management or Care Management within two years.
