The Care Navigator for Population Health Services Organization (PHSO) responsibilities include:

• The Care Navigator PHSO is an embedded member of a collaborative practice model that includes primary care clinicians and office staff, patients, caregivers, the PHSO and the community.

• They will develop and maintain relationships with high-risk members acting as the contact point in navigating their health and social needs in collaboration and partnership with the physicians and PHSO clinical programs. This will include a strong Health Coaching component, where transparent and motivational discussions incentivize patients to engage in the programs and resources offered to them.

• The Care Navigator will also assist members with community resource navigation by researching, finding, and linking patients with resources, services, and support mechanisms for their care plan and self-management needs.

Position details:

• Location: Hybrid – 3-4 days onsite, 1-2 days remote – Tampa, FL

• Status: Full Time

• Schedule: Monday - Friday 7:00 AM – 3:30 PM (flexible)

• Weekend Requirement: Occasional

• On Call: No

Required Education

• LPN OR bachelor's degree in social work (or related field)

Required Credentials

• LPN (Licensed Practical Nurse) licensure in lieu of bachelor's degree in social work (or related field)

Required Experience

• 3 years' experience in Care Coordination, Health Coaching, and/or Physician Office AND 1 year experience in health care
