It is anticipated, based on the referrals received, that these individuals may be involved in numerous incidents, have significant behavioral challenges and/or potential medical needs.• There are significant care planning needs (monthly) for the individuals to transition out of the CATH homes which begins right the time of admission. The LMSW will ensure that timely intake, discharge, and planning needs are met for these individuals.• LMSW will help ensure the referral process is complete and moving forward in a planful manner. The CATH homes will have a dedicated clinical team, which will also require additional training and support for the team.• The LMSW will be required to maintain the person-centered perspective to ensure the safety of all the staff and individuals at the home.• Community programming may be difficult for these individuals, which means more time in the home and the LMSW will help ensure community activities are being provided as per HCBS standards.• LMSW may serve as a primary point of contact and resource between the individual, their circle of support (including residential team members, family members and care manager).• Crisis intervention• Assist with helping individuals access needed services• Additional duties as assigned by TTL
