The Manager, Outpatient Care Management serves in an expanded nursing role to collaborate with and lead the outpatient care management process. This includes supporting technology solutions across the clinical programs within and interfacing with St. Luke's Physician Group practices and their aligned RN and Social Worker Care Managers. Integrates evidence-based clinical guidelines, preventive guidelines, protocols, and other metrics in supporting the development of treatment plans that are patient-centric. Promotes quality and efficiency in the delivery of healthcare for the populations served by the Care Managers.
JOB DUTIES AND RESPONSIBILITIES:
Assumes responsibility/accountability for planning, organizing, and managing the functions necessary to facilitate care coordination, resource utilization, and quality outcomes
Performs direct administrative functions for the department including, but not limited to payroll, staffing/scheduling, organization of monthly staff meetings, ensuring yearly evaluations, competencies, and educational requirements are completed.
Provides staff with ongoing necessary information, education, support, and coaching to foster an environment of knowledge, critical thinking, and accountability.
Works closely with appropriate practice, organizational and network leadership and providers in the development and attainment of departmental, organizational, and network goals.
Monitors and manages data pertinent to departmental goals including, but not limited to SDOH, readmissions, referral completion and outcomes, patient experience, and employee engagement and develops and implements solutions to address barriers to achieving goals.
Assures that care management services are provided consistent with the requirements of regulatory agencies and internal policies and procedures.
Maintains operational budgets, productivity, and fiscal responsibility in collaboration with finance.
Promotes patient engagement and addresses issues in accordance with policy on complaints and grievances.
Demonstrates competency in the assessment, range of treatment, knowledge of growth and development and communication appropriate to the age of the patient treated.
PHYSICAL AND SENSORY REQUIREMENTS:
Requires sitting, standing, and walking for extended periods of time (up to 8 hours at a time) Requires continual use of fingers for patient care, writing and handling patient charts. Routine use of upper extremities: occasional requirement to lift items up to 25 pounds. Regularly requires ability to stoop, bend, reach above shoulder level, and climb stairs. Requires the ability to hear normal conversation and good general, near and peripheral vision.
EDUCATION: Registered Nurse or Social Worker with current license to practice in the State of Pennsylvania or actively seeking licensure (RN or LSW/LCSW). NJ RN or LSW licensure required for NJ or seeking licensure through state reciprocity. Bachelor of Science in Nursing required within 5 years. Advanced degree and Case Management Certification preferred.
TRAINING AND EXPERIENCE: 5-7 years of care management experience in a health care setting required and 1-2 years of leadership experience preferred. Excellent leadership skills. Critical thinking skills. Knowledge in the management of highly complex psycho/social issues, utilization management, and care coordination.
