Wage: $22.26 - $26.50 per hour, depending on experience Benefits: Medical/Dental FSA 403(b) Retirement Additional Voluntary Benefits Paid Holidays Vacation/Sick Leave Education Assistance Program Employee Assistance Program Longevity awards Position Summary: Under the direction of the Jail Services Manager, the Case Manager Jail Transition Specialist provides case management and other various clinical services as needed. This position will interact with corrections staff and assist in the process of release planning. They will utilize the Sequential Intercept Model as language when discussing services and defining how it fits into the system of care. This position and program will be a part of care at Intercept(s) II-V. The schedule for this position is Monday-Friday 8am-5pm, with extensive travel throughout the Catholic Charities service area of Chelan, Douglas, and Grant Counties and occasional travel possible to Okanogan County. Responsibilities: Provides quality mental health services to consumers with a mental health disorder who are in the criminal justice system within the Catholic Charities service area. Services may be provided in a jail or juvenile detention setting, in the community, or in our Wenatchee office Services may include crisis intervention, assisting with applying for benefits, activities of daily living, psychosocial support, service coordination, linkage, transportation, medication monitoring, behavioral health counseling, accessing community resources, and housing. Establishes and maintains working relationships with community resources, coordinates consumer treatment with other providers, and initiates referrals to medical providers and other community resources when appropriate. Completes progress notes and other required documentation in the electronic consumer files within the required timeframe. Performs other duties as assigned Qualifications Job Requirements: The following requirements are those that are normally required for performance of this position. Physical Requirements: This position normally requires the physical demands of standing, walking, bending, lifting or performing other work requiring low physical exertion, talking and hearing on a regular basis to perform the job requirements. These physical demands are required up to 80% of the time. Position also requires ability to drive with adequate vision and skill. This position entails extensive driving within the Catholic Charities service area, which currently includes Chelan, Douglas, and Grant Counties, with possible contacts in Okanogan County. Non-Physical Requirements: Education Associate degree in behavioral sciences, criminal justice, social work, or related field required. Bachelor's degree preferred.
