POSITION PURPOSE

As a Registered Nurse, you will provide vital oversight and nursing care to our participants, ensuring high-quality service delivery and support for their health needs.

WHAT YOU WILL DO

  • Collaborate with interdisciplinary teams to develop and implement care plans.

  • Conduct assessments, manage treatments, and provide skilled nursing care.

  • Educate participants and families on health topics and self-care interventions.

  • Maintain accurate medical records and documentation.

  • Participate in quality improvement initiatives.

  • Support new participants in integrating into the PACE program.

  • Provide on-call coverage and assist in various program activities as needed.

MINIMAL QUALIFICATIONS

  • Graduate of an accredited Nursing program; BSN preferred.

  • Current, unencumbered RN license in the state of practice.

  • Active CPR certification required.

  • Minimum of 2 years of experience with the geriatric population.

  • Valid driver’s license and proof of insurance.

  • Strong clinical skills, attention to detail, and problem-solving abilities.

  • Excellent communication skills and proficiency in Microsoft Office.
