Description
TheHousing Case Manageris responsible for the provision and coordination of home visits, social service referrals, entitlements management and housing stability management for up to a 25-client caseload. The Housing Case Manager is responsible for ensuring compliance with contract services and ensure all clients are engaged in health care and additional services based on client need.
ESSENTIAL JOB FUNCTIONS
The following duties are mandatory requirements of the job:
Maintain a caseload of up to 25 single individuals and identified family members
Conduct at least one monthly home visit to the clients who reside in New York City
Provide a range of case management services including but not limited to counseling, health care linkages, advocacy, entitlements management etc
Complete all required paperwork by established deadlines. Paperwork includes quarterly service plans, assessments and reassessments, at minimum monthly progress notes, home visit documentation, monthly apartment inspection forms and all other documentation as assigned
Provide coordination of care with medical and mental health providers
Make all needed referrals to GMHC and outside providers
Meet all contract deliverable services (home visits, advocacy, escorts, groups, counseling sessions etc)
Data enter all deliverables into eCOMPAS, TREAT and time and effort reports per contract mandates
Ensure clients have a strong understanding of the Terms and Conditions of Tenancy
Assist client to maintain their housing permanence by monitoring and collecting monthly rent payments
Provide escorts to health care providers, entitlement resources, housing management resources and other providers as assigned
Provide visitation to hospitals or other locations in order to maintain client connection
Attend and moderate case conferences with client and providers
Connect all clients to services at GMHC and make referrals to departments based on need
Attend all GMHC meetings and events as assigned
All other duties as assigned
Requirements
EDUCATION AND CERTIFICATION
High School Diploma, GED or HSE and at minimum 2 years’ experience working with clients with a history of housing related issues
Associate or bachelor’s degree preferred
SPECIAL SKILLS AND KNOWLEDGE
In addition to the above-listed job responsibilities and educational requirements, the ideal candidate for this position possesses most or all of the following:
Knowledge of housing resources related to housing stability
Extensive knowledge utilizing Microsoft Office Suite
Ability to work within a fast-paced, data-driven, performance-based environment
Exceptional written and oral communication skills with the ability to interface with a variety of internal and external colleagues
Extensive experience working with individuals with HIV/AIDS, substance use issues and mental health needs
Demonstrated ability to work effectively as part of interdisciplinary team
Bilingual in English and Spanish a plus

This is a Hybrid position which requires some days in office
Salary Description
$45,000 - $50,000 per year
