Williamstown Recovery Operator, LLC Case Manager - Weekends & Weekdays in Williamstown, Massachusetts

A Case Manager is responsible for providing support, guidance, and case management services to individuals at Greylock Recovery. This role involves developing personalized recovery plans, coordinating services, and ensuring that clients receive appropriate care throughout their stay. The ideal candidate will have strong communication and organizational skills, a passion for helping individuals recover from substance use disorders, and the ability to work collaboratively with a multidisciplinary team.

KEY RESPONSIBILITIES:

Client Assessment & Intake: - Conduct thorough assessments of clients\' physical, emotional, social, and vocational needs at intake. - Develop individualized treatment and recovery plans based on assessment findings and client goals. - Maintain accurate and up-to-date client records and case notes in accordance with facility policies. Case Management: - Provide ongoing support and guidance to clients throughout their stay in the program. - Coordinate client services, including medical, psychiatric, vocational, and social support services. - Ensure clients have access to necessary resources such as housing, employment opportunities, and aftercare programs. Collaboration & Advocacy: - Work closely with a multidisciplinary team, including therapists, doctors, social workers, and counselors, to ensure integrated care. - Advocate for clients to ensure they receive appropriate services and treatment within and outside the facility. - Act as a liaison between clients and external service providers, including legal, housing, and healthcare resources. Monitoring & Reporting: - Monitor clients\' progress throughout treatment and adjust care plans as necessary. - Conduct regular progress reviews and provide feedback to clients on their recovery journey. - Prepare and submit reports on client progress, program outcomes, and case management activities to supervisors and relevant stakeholders. Discharge Planning & Aftercare: - Develop comprehensive discharge plans that include referrals to outpatient services, support groups, and community resources. - Provide follow-up support to clients after discharge, including periodic check-ins and assistance with accessing aftercare services. - Help clients develop relapse prevention strategies and connect with long-term recovery resources. Compliance & Ethical Responsibilities: - Ensure compliance with all federal, state, and local regulations related to substance use treatment. - Maintain confidentiality and adhere to ethical standards of care. - Participate in staff meetings, training sessions, and continuing education to stay current on best practices in substance abuse treatment.

QUALIFICATIONS:

Education: - Bachelor\'s degree in Social Work, Psychology, Counseling, or a related field preferred. - Master\'s degree or clinical licensure (e.g., LCSW, LPC, CADC) preferred. Experience: - Minimum of 2-3 years of experience in case management or counseling, preferably in a substance abuse or mental health setting. - Experience working in a residential treatment facility or similar setting is highly desirable. Skills & Abilities: - Strong knowledge of substance use disorders, treatment modalities, and recovery support services. - Ability to build rapport with clients from diverse backgrounds and maintain a compassionate, non-judgmental attitude. - Excellent organizational skills with the ability to manage multiple cases simultaneously. - Proficiency in using case management software and maintaining detailed records. - Strong written and verbal communication skills.
