CW Solutions has an immediate opening for a Housing Case Manager to provide services in our PATHS Program. This position is responsible for providing comprehensive case management services under a Housing First, Rapid Re-Housing model to youth/emerging adults who: have previous child welfare intersection, are between the ages of 17-21, and are in our southern service delivery area in Adams, Portage and Wood counties. This position will require travel to all three counties; however, the assigned office location is Wisconsin Rapids. This is a full-time, grant-funded position. There is no standard remote work time with this position as it requires in-person service delivery. The Housing Case Manager position has two core responsibilities: Case Management: Empower youth and emerging adults through person-centered advocacy to assist participants in identifying their housing and other goals, based on their individual wishes, strengths, talents, abilities and needs. Develop and maintain a written action plan based upon the identified goals and needs and provide the support and follow-up services needed to address the range of challenges participants confront to sustain safe and stable housing. Community Relationship Development: Develop and sustain integrated relationships with landlords, housing programs, community resources and other agencies for community-building and program participant wraparound support. For a full job description, go to https://www.changewithin.net/employment-opportunities ***** OTHER EXPERIENCE AND QUALIFICATIONS: *Must have a servant heart with an internal motivation to serve others. *Sustain a non-judgmental, empowering approach to all youth and emerging adults, including providing continuous opportunities for engagement and re-engagement. *Ability to work positively when participants express reactive behaviors and actions. *Apply trauma-informed care, inclusion and allyship in all aspects of programming and housing supports. *Proven background experience or knowledge of working with youth/emerging adults who may express heightened trauma response. *Strong vision of hope and positive expectation and outlook when engaging all parties involved. *Skilled in applying trauma informed care practice in daily work. *Case management skills, including motivational interviewing, family systems, plan development and positive accountability (able to be trained on-the-job). *Ability to work independently with strong sense of focus, task-orientation, nonjudgmental approach, open personal qualities, and a clear sense of boundaries. *Ability to work in a variety of settings with culturally diverse families and communities with the ability to be culturally sensitive and appropriate, supporting participants within the context of their identity, family, community, tribe, history, culture and traditions. *A strong sense of and respect for confidentiality involving both participants and fellow employees. ***** APPLICATION INSTRUCTIONS: Apply Online: https://form.jotform.com/CWadministration/cws-employment-application
