POSITION TITLE: Behavioral Health LPN Unity House

DEPARTMENT: Intensive Residential Treatment Facility

GENERAL HOURS

OF WORK: Weekdays, Weekends, Holidays, Days, Evenings

EXEMPT/NON EXEMPT: Non-Exempt

SUPERVISOR: Treatment Director

Position Summary :. The Behavioral Health LPN will provide direct care under the direction of the Treatment Director/RN Manager in regards to training, treatment, support, and activities within a residential treatment facility that provides behavioral health services to individuals that require medical monitoring, mental health assessing, implementing of intervention strategies, treatment goals and environmental modifications that will support an individual’s ability to transition back into the community.

Education, Experience, and Skills Required :

  • Valid LPN licensure in the state of MN

  • CPR/First Aid Certification or attain within 30 days of employment

Essential Functions :

  1. Utilize nursing knowledge and skills in the safe implementation of therapeutic and rehabilitative nursing care of clients as evidenced by observation and documentation of treatment goals, mental health assessment and positive patient outcomes.

  2. Perform delegated nursing duties using established procedures, policies, guidelines and standards as observed by the RN Nurse Manager.

  3. Complete vital signs and perform ongoing mental health assessment every shift.

  4. Maintain and coordinate communication among client, staff and RN manager as evidenced within medical record, documentation and health care team.

  5. Assist with client admissions and discharge.

  6. MAR audits and compliance, including narcotic count, disposing of medication according to policy.

  7. Administer medication accurately, observing client response, and providing documentation in the medical record.

  8. Assuring that effective communication exists with patients as reflected in the plan of care. Give change of shift report consisting of pertinent and relevant information for assigned clients.

  9. Work collaboratively with other members of the health care team in coordination of the clients care.

REQUIRED PHYSICAL ABILITIES:

  1. While performing the duties of this job, the employee is regularly required to stand; use hands to finger, handle, or feel; and talk or hear.

  2. The employee is occasionally required to sit and climb or balance.

  3. Must be able to sweep, mop, shovel, and lift up to 40 lbs.

  4. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.

REQUIRED COMMUNICATION/MATHEMATICAL/REASONING SKILLS:

  1. Must possess the required level of skills and abilities to read, write, speak, and apply mathematical concepts and sound reasoning to successfully perform the essential duties and responsibilities of this job.

  2. Must be able to perform basic operations on a computer within the Windows operating system.

  3. This position requires problem solving skills, critical thinking skills, and the ability to multi-task.

WORK ENVIRONMENT:

  1. May be exposed to communicable diseases when working in a health care environment.

  2. The employee is occasionally exposed to wet and/or humid conditions and outside weather conditions.

  3. The noise level in the work environment is usually moderate.
