Special Citizens Futures Unlimited Day Services Counselor in Yonkers, New York
JOB SUMMARY: As a Day Services Counselor, you will be assisting individuals with Autism to acquire, retain or improve their socialization, communication, and travel skills through recreational activities and community outings. The Day Services Counselor should have experience working with a diverse group of individuals. The employee will be assigned on a daily basis to work with various individuals and supervise developmentally disabled individuals in developing skills as outlined in prescribed treatment plans. Data collections and other record-keeping is required. The employee will have contact with family members, neighbors, community vendors, and service providers. As part of this responsibility, the Day Services Counselor ensures that all measures necessary for the safety and well-being of the consumer are maintained at the highest possible level at all times. PRIMARY RESPONSIBILITIES: To assist in the planning and preparation of activities in and outside of the program under the supervision of the program coordinator.To accurately communicate and complete all documentation for program plans, reportable events and incidents within established reporting periods. Day Services Counselors are responsible for producing accurate and clearly written/entered documentation of the services. Service records are to be completed daily at the end of each shift. Any documentation that is incomplete or incorrect must be corrected upon finding. To serve as an appropriate role model and engage in interactions, which promote and insure the dignity and rights of all participants, service providers, and co-workers. To maintain professional relationships and composure with the consumers. To ensure the safety of the consumers at all times. Conducts fire drills following the fire evacuation plan regularly. Teaches and reinforces use of a variety of communication skills. Maintains confidentiality of information. To immediately report any and all allegations and information relating to possible or actual abuse/ neglect to the appropriate parties. To provide direct care assistance and agency approved transportation to individuals in the Day Services program. Actively participates at staff meetings by communicating observations concerning progress and reaction to various therapy programs. Takes, records, reports temperatures, blood pressures, pulse rate, weight, and other significant symptoms/health-related occurrences; aids developmentally disabled individuals in understanding, recording, and reporting such factors. Adheres to each supported persons staff action plan, behavior support plan safeguards. Teaches/supervises/participates in a variety of leisure and recreational activities, crafts projects, seasonal and permanent home decoration. To carry out the implemented goals described in the Staff Action Plan. To participate in agency in-services, meetings, and OPWDD trainings, as required. To maintain all appropriate job related certifications. To review, understand and abide by all policies and procedures. Granted access to EPHI based on job title and work location. Comply with applicable legal requirements, standards, Special Citizens policies and procedures including, but not limited to Compliance Program: Code of Business Conduct, HIPPA, and documentation Standards. MANDATORY QUALIFICATIONS: High school diploma or GED required. Must be at least 18 years of age. Experience working with aggressive individuals with behavior problems is preferred. Maintenance of necessary certifications as per OPWDD and Agency Regulations. Valid unrestricted NYS Drivers License, to agency standard. Ability to travel and transport individuals. Ability to demonstrate competency in written, verbal and computational skills to document services in accordance with program and OPWDD mandates. Flexibility as days, hours and location may change based on agency needs. Successful completion of criminal background history and fingerprint examination.
